Event Investigation

The main reason for investigating events is to try and identify the root cause & associated causal factors that contributed to the unwanted event occurring, in order for the organisation to have greater opportunity to prevent the same type of event from potentially occurring again.

 

Determining the underlying facts of an unwanted event will also assist in identifying control measures that can be put in place to prevent further re-occurrences. It also provides a mechanism to unsurface other unidentified hazards associated with the business. It is equally important to communicate the outcome of any event investigation company wide, that way all interested parties are made aware of the potential risks and of changes the business has made to a process or procedure, including the consultative reasons behind those changes.

 

Additionally, timely and efficient reporting on hazards, incidents, and other occurrences with actual or potential risk to adversely affect the “coordinated activities to direct and control and organization with regard to risk” are applied in order to mitigate “the effect of uncertainty on objectives” aligning with the risk profile of the company.
